Continuing students and new students will need to fill out the following:
New students will also need to provide the following:
A copy of student’s birth certificate
Complete a physical with the student’s physician: Physical Exam
A record of immunizations: Oregon State Immunization Information
A records request from previous school attended: Records Request
We also ask that all parent’s/guardian’s fill out our Volunteer form. It is important that we are able to work as a team with you to provide the best experiences for your child!
